Operational Activities: We may collect, use, and disclose personal information for risk management, security, or to improve the quality of Crystal Shore Physiotherapy services, including evaluation and monitoring. For instance, we may record phone calls at our after-hours call center for training, documentation, and quality assurance. We may also conduct video recording for security purposes in public areas of our facilities, such as reception, entrance and exit locations, and outdoor areas. Crystal Shore Physiotherapy may engage in research and analysis, where your personal information, including health information, may be analyzed in a de-identified form. The results will not be published or disclosed in a manner that could identify individuals. Any additional research involving identifiable information will require separate informed consent.

We may also disclose information for certain proceedings and to investigate or prevent actual or suspected harm to persons or property.
